‘Beyond “Detox”: Using Pharmacogenetics to Understand Drug Activation, Clearance and Biotransformation’

Detoxification is frequently discussed as a generalised process of “removing toxins”, yet clinically it represents a coordinated system of Phase I biotransformation, antioxidant protection and Phase II conjugation that also determines how many medications are activated, metabolised and eliminated.

This presentation uses an anonymised female case study to demonstrate how genetic data can provide additional context for individual variation in drug response.

The profile identifies potentially reduced CYP2D6 activity, with implications for reduced activation of prodrugs such as codeine and tramadol, while medications dependent on CYP2D6 for clearance may instead achieve higher exposure.

A CYP2C19 intermediate metaboliser profile provides a contrasting example, where reduced enzyme activity may decrease activation of clopidogrel while slowing clearance of medications including proton pump inhibitors, selected antidepressants and diazepam – particularly important with a family history of depression.

The case also explores how Phase I metabolism must be considered alongside downstream metabolic capacity.

Variants affecting SULT1A1 sulphation, GSTM1 glutathione conjugation and UGT1A6 glucuronidation illustrate how reactive intermediates and drug metabolites ultimately require effective antioxidant buffering and Phase II elimination.
Rather than using genotype to dictate prescribing, the presentation proposes a pathway based approach integrating pharmacogenetics with medication history, drug-drug and drug-nutrient interactions, clinical response and appropriate monitoring.

The aim is to demonstrate how genetic information can help explain unexpected efficacy or adverse effects of prescription medication while supporting safer, more individualised medication use.

About Dr Dean St. Mart

Dr Dean St Mart PhD is a fitness industry renowned Pharmacologist and leading Educator as the Product Formulator for the UK based supplement brand called Supplement Needs.

He holds a Double 1st Class Honours degree in Chemistry and Pharmaceutical Chemistry from the National University of Ireland Maynooth where he finished top of the university, and holds a PhD in Synthetic Organic Chemistry alongside being a Certified Sports Nutrition Coach (CSNC) and Naturopathic Practitioner.

He is an Author for the National Academy of Sports Medicine (NASM); as the Subject Matter Expert (SME) for Dietary Supplements and Performance Enhancing Drugs within their Bodybuilding and Physique Coach Qualification Curriculum.

His interests lie in pharmacology, novel drug design, health and sports dietary supplementation, and applying Functional Medicine to athletes to improve their health. His love of Functional Medicine is what led to the creation of the Dr Dean supplement range within the Supplement Needs brand; in order to provide a one-stop shop for consumers to purchase products that are dosed efficaciously backed by clinical research, all fully transparent, and without any hidden proprietary blends.

‘Mould & the Cell: Building Resilience from the Inside Out’

Mould exposure is often approached through avoidance and detoxification, but understanding the cellular response to environmental stressors adds an important layer to the clinical picture. In this webinar, Dr. John Kim, Pharm.D., FAARFM, will explore mould through a cellular health lens, connecting environmental exposures with mitochondrial function, cell membrane health, redox balance, nervous system regulation, gut health, and endogenous detoxification pathways. He will discuss strategies designed to support normal cellular function and resilience, with an emphasis on building physiological capacity and supporting the cellular foundations that help the body adapt, regulate, and restore balance.

About Dr. John Kim, Pharm.D., FAARFM

Dr. John Kim, Pharm.D., FAARFM, is a functional medicine pharmacist, educator, and founder of Kim Wellness and the ENCORE Method™, a whole-cell framework centered on environmental health, nervous system regulation, cellular function, detoxification, microbiome health, and resilience. A graduate of Rutgers University’s Ernest Mario School of Pharmacy with advanced fellowship training in functional medicine through A4M, John brings a systems-based approach to understanding how environmental exposures, mitochondrial function, the microbiome, and nervous system regulation intersect to influence health. His work focuses on helping practitioners look beyond isolated symptoms and support the broader physiological terrain that contributes to cellular resilience and overall well-being.
